Form to collect information from students/parents who submitted requests to enroll and have not as yet.

  • Earlier this year, your student made a request to enroll in SmartDrive Classic online defensive driving program. SmartDrive sent an email to your student on February 1st indicating the program was now open for enrollment. Would you please complete this brief survey on your student's enrollment progress and let us know of any difficulty you or your student may have encountered in enrollment or in undertaking the program? Thank you. The SmartDrive Foundation
If you choose not to have your student enroll in, or engage in any SmartDrive online programs, mark the button above and scroll to the bottom of this form and click submit. The student information will be removed from SmartDrive databases and you will receive a confirmation message.
It is a state requirement that all students must already hold a valid driver's license or permit to be allowed to enroll in SmartDrive Classic
SmartDrive follows Child Internet Protection Act guidelines requiring parental notification for minors to engage in our online programs where personal information about the student is required for enrollment. State regulations require SmartDrive to collect detailed driver's license/permit information before a student can enroll in this Defensive Driving program. SmartDrive will not ask a minor to directly provide this information, and if there is no positive parental parental permission given, that student will not be allowed to enroll.
In order provide assistance, we will need the Parent/Guardian email address and telephone number. Please also include a time when it would be best to call you.
